National award checklists

Commanding officers use these checklists to make sure every required document is in each award package sent to the Division. Missing documents may disqualify a candidate.

  1. Navy League Award Application Form, NL (104) ERequired
  2. Fortress Cadet Information CardRequired
  3. Photographs of the nominee in full uniform: one head and shoulders, one full body with boots in viewRequired
  4. School marks
  5. Letters of recommendation from
    • Commanding Officer (mandatory)
    • Branch President (mandatory)
    • School Principal
  6. One or more letters from
    • Teacher
    • Minister
    • Coach
  7. Any additional recommendation letters showing involvement in the community and citizenship. These help for the Orson Burgess Award.
  8. An autobiography written by the cadet. It can be typed, but must be signed by the cadet and include:
    • Brief information about the cadet
    • Plans after leaving Navy League
    • School activities
    • Cadet activities
    • Organized sports
    • Activities outside of cadets and school
    • Benefits from being a Navy League Cadet
  9. The Commanding Officer’s letter must include:
    • Attendance record
    • Exam results
    • Promotions and dates
    • Awards received
    • The cadet’s participation within the corps and the community
    • General comments about the cadet
  10. Original documents, with one photocopy, go to your branch, which reviews them and forwards them to the Divisional Awards Chair. The Division forwards them to the National Office.

The more information and letters of recommendation you provide, the better your nominee’s chance of being selected. A video can be included. Branch presidents and commanding officers are responsible for completing award packages properly before they are sent.
